A. Please take note of the following provisions which were clarified during the Pre-Bid Conference, to wit:

1. In addition to the Bidding Document Requirements, bidders shall submit the following:
Eligibility Requirements
Class “A” Documents:
ITB 13.2 (a5) Legal Documents

• Registration Certificate of all chainsaws to be used in the project (whether owned or rented) issued by the Department of Environment and Natural Resources (DENR).

ITB 13.2 (a6) Technical Documents

• Contractor shall submit proven track record of experiences with at least one (1) project for the past 2 years in the field of forestry or similar/related tasks in the form of a Certificate of Completion and Acceptance by their client.

Class “B” Documents:

• Certificate of Site Inspection, duly notarized

Technical Bid/Proposal

• Proposed Methodology for the actual execution of the services, includes the tools/equipment, manpower deployment/schedules, work plans etc.

Financial Bid/Proposal

• Bids shall be exclusive of VAT.

2. Project Duration:

• Sixty (60) calendar days from receipt of the Notice to Proceed.

• Payment Terms:
• Within 15-30 days from acceptance by SBMA of completed performance/ delivery of the services.

• Ten percent (10%) Retention Fee

Shall be released one (1) year, from acceptance by SBMA of the completed delivery and performance of the services.

B. Bids should be submitted on August 16, 2006, Wednesday, 2:00 p.m. at PPMD Conference Rm., Bldg. 255, Barryman Road, SBFZ. The opening thereof shall follow immediately thereafter.
