A. In addition to the bidding requirements below documents shall likewise be submitted during the submission of bids;

Eligibility Requirements
·   Clause 13.2 (a5) (under Class “A” Documents)
Other required documents:
License to Operate/Authorization to sell ammunition issued by Philippine National Police (PNP), Camp Crame.

·   Clause 13.5 (under Class “B” Documents)
Duly signed certification under oath that each of the documents submitted in satisfaction of the eligibility requirements is an authentic and original copy, or a true and faithful reproduction of the original, complete, and that all statements and information provided therein are true and correct.

Please refer to the attached Bid Documents Checklist of your reference and guidance.

Technical Bid/Proposal
Specification Requirements:
·   Please comply with the notes under Section VII of the Bid Documents.

Financial Bid/Proposal

• Bids shall be exclusive of VAT.

Delivery Requirements:
·   Within Fifteen (15) calendar days upon receipt of Purchase Order by the winning bidder.
·   Supplier shall submit Sales Invoice during the delivery of the items.

Note: Clause 6.2 of the Special Conditions of the Contract refers only to the items procured from abroad or imported goods/items.

·   Within Ten percent (10%) Retention Fee shall be released three (3) months, from acceptance by SBMA of the delivered items.

Payment Terms:
·   Within thirty (30) calendar day from the acceptance by SBMA of the delivered items.

B.   The submission/opening of bids is on August 16, 2006, 2:00 p.m. at PPMD Conference Rm., Bldg. 255, Barryman Road, SBFZ.
